The Fall Risk Assessment Program overview is designed to introduce outside organizations to our Fall Risk Assessment Program.
It comes with the following materials:
- an introductory letter that can be altered to fit your specific needs and sent with the rest of the materials
- a sample flyer to show that they do not need to design the flyer themselves
- a detailed overview of the program and what they can expect
- a carbon copy screening form to be used during the screenings
