In exchange for participation in the picnic activities organized by Amberwell Health at Mission Lake Christian Camp, including inflatables, carriage rides, petting zoo, and kids train, I agree for myself and (if applicable) for the members of my family, to the following: I agree to observe and obey all rules and warnings, and further agree to follow any oral instructions or directions given by representatives of Amberwell Health. I recognize that there are certain inherent risks associated with the above activities and I assume full responsibility for personal injury to myself and my family members and further release and discharge Amberwell Health for injury, loss or damage arising out of my or my family’s use of or presence whether caused by the fault of myself, my family or other third parties. I agree to indemnify and defend Amberwell Health against all claims, causes of actions, damages, judgements, costs or expenses, including attorney fees which may in any way arise from my or my family’s use of or presence upon the activities provided at the picnic.
